Washington Consumer Health Data Privacy Policy

A Haven for Us maintains this Consumer Health Data Privacy Policy in accordance with Washington’s My Health My Data Act. This policy describes the consumer health data we collect, the purposes for which we collect and use it, the sources from which we receive it, the circumstances in which we share it, and how consumers may exercise their rights.

Categories of Consumer Health Data We Collect and How We Use It

Depending on the program or service requested and information an individual chooses to provide, A Haven for Us may collect the following categories of consumer health data:

  • Information concerning physical or mental health conditions, status, symptoms, treatment, or needs

  • Information concerning psychological, behavioral, medical, or health-related services or interventions

  • Information concerning healthcare procedures or health-related expenses

  • Information concerning prescribed medications or medication-related expenses

  • Information concerning disability, chronic illness, or health-related accessibility needs

  • Information concerning reproductive or sexual health or healthcare

  • Information concerning HIV-related services, prevention, treatment, or expenses

  • Information concerning gender-affirming care, services, procedures, or expenses

  • Information identifying or relating to an individual's request for healthcare services, health-related resources, or financial assistance for a health-related purpose

A Haven for Us may collect and use these categories of consumer health data to:

  • Provide or administer a program or service requested by the consumer

  • Respond to resource navigation requests

  • Provide peer support programming and accommodations

  • Determine eligibility for applicable programs

  • Review and administer applications for financial assistance

  • Evaluate requests for Community Microgrant funding

  • Communicate with applicants or program participants

  • Facilitate referrals or assistance requested by the consumer

  • Verify information when reasonably necessary to administer the requested program

  • Distribute financial assistance

  • Maintain appropriate organizational and program records

  • Comply with applicable legal or regulatory requirements

  • Evaluate and improve A Haven for Us programs and services

Categories of Sources of Consumer Health Data

A Haven for Us may collect consumer health data from:

  • The consumer directly

  • Applications, registrations, intake forms, requests for assistance, surveys, or other forms completed by the consumer

  • Communications between the consumer and A Haven for Us

  • Documents or supporting materials voluntarily submitted by the consumer

  • A person assisting the consumer at the consumer's request

  • A third party when the consumer directs or authorizes us to obtain information from that third party

Categories of Consumer Health Data We Share

Depending on the program or service requested, A Haven for Us may share:

  • Consumer health data contained in an application when authorized program reviewers reasonably require that information to evaluate the application

  • Consumer health data reasonably necessary for a service provider to host, store, transmit, secure, or otherwise process information on behalf of A Haven for Us

  • Consumer health data reasonably necessary to facilitate a referral, resource request, or other service specifically requested by the consumer

  • Consumer health data that disclosure is required or permitted by applicable law

Categories of Third Parties and Specific Affiliates With Whom Consumer Health Data Is Shared

Consumer health data may be shared, as applicable, with the following categories of third parties:

  • Authorized application or program reviewers

  • Website or online-form service providers

  • Cloud storage, database, or customer relationship management service providers

  • Email, SMS, or communications service providers

  • Video conferencing or virtual-program service providers

  • Information technology and security service providers

  • Payment providers when information is necessary to provide financial assistance

  • Organizations, providers, or resources with whom the consumer asks A Haven for Us to communicate

  • Governmental authorities or other recipients when disclosure is required or permitted by applicable law

Specific affiliates: A Haven for Us does not currently share consumer health data with any affiliated legal entity.

How to Exercise Your Consumer Health Data Rights

Washington consumers have the right to:

  • Confirm whether A Haven for Us is collecting, sharing, or selling consumer health data concerning them and access that data

  • Obtain information concerning third parties and affiliates with whom their consumer health data has been shared or sold

  • Withdraw consent from future collection or sharing of consumer health data where applicable

  • Request deletion of consumer health data concerning them

  • Appeal a refusal by A Haven for Us to act on a consumer health data request

Please identify the right you wish to exercise but do not include sensitive health information in your initial email. A Haven for Us may request additional information reasonably necessary to authenticate your identity and will provide further instructions if sensitive information is needed.

Washington's current statute provides those access, withdrawal, deletion, authentication, response, and appeal rights.
