Consumer health data may be collected through online forms, program registrations, applications, communications, supporting documents, resource navigation requests, or other interactions initiated by the consumer.
Consumer health data may be stored or processed through systems used by A Haven for Us and authorized service providers.
Access is limited to authorized individuals, reviewers, contractors, or service providers whose access is reasonably necessary to perform authorized responsibilities.
A Haven for Us does not sell consumer health data and does not use consumer health data for targeted advertising.

A Nevada consumer may request that A Haven for Us:

Please identify the request you wish to make but do not include sensitive health information in your initial email. A Haven for Us may request additional information reasonably necessary to authenticate your identity.
Nevada law generally requires authenticated consumer requests to be addressed without undue delay and within 45 days, subject to a permitted extension in appropriate circumstances. Deletion requests are subject to the procedures and timelines established by Nevada law.
Reviewing or Requesting Changes to Consumer Health Data
A Nevada consumer who believes consumer health data maintained by A Haven for Us is inaccurate may request that A Haven for Us review or correct the information.
